+1 Jake's a very responsible person who deserves this rank, he is good help to everyone he meets, and is a overall good guy and friend.

+/- I agree with @QwalFee since you are a bit short tempered with some players. Also, I haven't seen you on teamspeak which is a requirement. If your not active on teamspeak, thus application is unlikely to be accepted. If I see you on teamspeak more often, I might change my reply.

-1, You have been banned three previous times also calling them "False bans" won't get you anywhere, secondly your responses are laid back and somewhat childish (Tip: If your trying to impress someone make it sound like that you actually care and want it), When application asks "Tell us about yourself:" the response you gave does not even answer the question. My suggestion to you is to edit this and re think some of your answers.
 
I may have quit TBB, but it doesn't mean I have fully, though when I have played on the servers, i've been with TBB for roughly 3 years aswell, in that time i've never seen you, aswell, on your app, you have missed a question.
